Please submit the form below and let us know how many masks you would like.  Your order will be ready the next day following your order during our summer office hours posted to the left unless we are closed for that day.

Please bring your check payable to St. Clare of Montefalco when you pick up your mask. If you have questions or prefer to order by phone, please call the parish office.  Thanks!
